The PI/QA documents and Standardized Operative Delivery Notes included in this section were developed by the Patient Safety Initiative of HCA. HCA has approved their distribution and use by any hospital or hospital system that wishes to incorporate these documents into a patient safety program to improve the outcomes of operative vaginal delivery, including vacuum-assisted delivery.
